Dementia Support Services
At Pendleside Hospice, we provide a variety of specialised support services for people, and their families and carers, living with dementia, including 12-week guided programs and personalised care from our Admiral Nurse.
Our services include:
- Weekly Dementia Support Groups
- Carer Support Groups
- Support with New and Early Diagnosis
- Practical Care
- Hospice at Home Support
- Family Support and Counselling
- Advance Care Planning Support
- Support with Symptom Control
- Support from Our Dementia Specialist Admiral Nurse
Any patient or carer who has been identified with complex needs will be referred to our Admiral Nurse, Rachel Moroney.

Our Dementia Strategy
Our dementia strategy for 2025 -2030 outlines our vision, mission and objectives for the next five years, and how we plan to deliver this.
This strategy has been developed by our Dementia Strategy Group, which includes; people with lived experience of dementia, employees, and volunteers of Pendleside Hospice. The following people and groups have also been involved with its development:

Making a Referral
Referrals will be accepted from any patient, relative, carer or health care professional and can only be made with your consent. You can speak to your nurse, social worker or GP who can call us or complete a referral form for you.
If you wish to speak with a member of the team first, please contact us on 01282 440 100. Alternatively, please visit our Make a Referral page, to download the referral form. Once you have filled out the form, please email this to [email protected].
If you are unable to complete the referral form, please call the Health, Wellbeing & Rehabilitation team on 01282 440 110 and they will complete the referral form for you, over the phone.
