Planning Ahead: The Importance of Advance Care Planning

Wednesday 7th May 2025 marks the UK’s first ever Advance Care Planning (ACP) Day — a national awareness day dedicated to encouraging conversations about future care.

It’s an opportunity to reflect on what matters most, to talk openly with those we trust, and to take steps toward making our wishes known.

At Pendleside Hospice, we’re proud to support this important initiative and help individuals and families feel confident about planning ahead.

We understand that discussing end-of-life care can be challenging. However, taking the time to consider and communicate your wishes through Advance Care Planning (ACP) can provide peace of mind for you and your loved ones.

What is Advance Care Planning?

Advance Care Planning is a process that allows you to think about and share your preferences for future care, ensuring that your values and wishes are respected if you're ever unable to make decisions for yourself. This might include:

  • Reflecting on what matters most to you.
  • Discussing your wishes with family, friends, and healthcare professionals.
  • Documenting your preferences in a written plan.
  • Reviewing and updating your plan as circumstances change.

Why is Advance Care Planning important?

  • It ensures that your care aligns with your personal values and desires.
  • It reduces stress and uncertainty for your loved ones during difficult times.
  • It guides healthcare professionals in making decisions that reflect your wishes.
