Hospice at Home We believe that hospice care for the people of Burnley and Pendle should extend far beyond the hospice building. Our aim and purpose is to promote the quality of life for each individual and his or her family and carers. We do this by delivering specialist palliative care, appropriate to each patient‘s needs and family circumstances. This encompasses a whole range of physical, psychological, social and spiritual needs.  Staying at home is a reality Hospice at home offers nursing, respite and personal care to patients, eighteen years of age or over, in their own home to complement existing services. The care is given by qualified nurses and experienced auxiliaries and is delivered according to each patient‘s needs and family circumstances. Hospice at home offers emotional care and support to patients, their family and carers. Hospice at home extends the hospice philosophy of care into the community. Hospice at home offers a night care service. Caring for someone is a full time job. Without help and support, carers may become exhausted and feel they can no longer cope. It can be particularly difficult if the person is very ill, as this places ever increasing physical and emotional strain on families and carers. Our hospice at home service has been developed for those who wish to continue to receive care at home and avoid unnecessary admission to hospital or the hospice. The service works together with voluntary and other services whenever possible. The period of care will begin at first contact and continue for as long as necessary. We will provide care based on assessment of individual needs. We realise that good communication between all concerned is essential and, at all times, we maintain respect for privacy, confidentiality, dignity, and patients‘ choice, regardless of age, culture or religious belief.  Referrals Referrals can only be made with the patient’s consent. Referrals will be accepted from any patient, relative, carer or health care professional. In urgent cases, a referral will be taken by telephone. In this case, it must be made directly to the nurse in charge of hospice at home. When referrals are received from patients, their carers or relatives, contact will always be made (with the patients consent) with the patients General Practitioner and any other relevant Health Care Professionals to obtain clinical information. Referrals are taken any day between 9.00am and 5.00pm Monday to Sunday. Medical care The patient’s own GP will continue to provide your medical care. The nursing staff can ask advice from the hospice Medical Director on the patient’s behalf if requested. The nurses will liase with other professionals who may be involved in the patient’s care, for example, district nurses, Macmillan nurses, social workers or the GP.
Complementary therapies The patient may be offered a complementary therapy if it is thought this will be of benefit to them.
